Cole Home Healthcare has recently begun voluntary participation in
HHQI (Home Health Quality Initiative).  We are proud to be a charter
member of the organization that focuses on reducing Acute
Hospitalizations by introducing programs aimed at keeping patients
from being admitted to the hospital emergently.  Several new programs
have been adopted that have had an immediate, noticeable impact on our
patient's quality of life as well as reducing their dependence on
Physician contact.  We strive on a daily basis to uphold the highest
level of care for our patients; and this new program not only supports
our mission, but has dramatic, immediate impact on patient care
outcomes.

As always, we wish to stress the importance of employing every person
that has patient contact directly by our Agency.  Other Agencies
contract the services of several people to provide different aspects
of patient care.  This leads to unstable provision of services as well
as a lapse in quality control measures. There is no better way to
promote the highest quality of patient care and improved outcomes than
by holding direct employees accountable, as well as having tremendous
care coordination.  Through our State Certified Outpatient
Rehabilitation Facility, we are able to provide great care across the
entire spectrum of Disease Management.  This facet of our organization
only seeks to increase our patient's comfort and continuity of care.
